To cancel a policy, send a written request, signed by the policyholder or the policyholder's legal representative, to the following address. If the policyholder's legal representative is signing, please include a copy of the appropriate legal paperwork.
To inform us of the death of a policyholder, send a written notification with a copy of the Death Certificate to the address above.
In the event of a policyholder's death, a refund may be owed for unearned premiums. Unearned premium refunds will be made payable to the estate of the deceased policyholder. To request that the refund be issued to a trust or an individual, please call us, and we will send you the appropriate affidavit.
Written cancellation requests and death certificates can also be submitted through the Policyholder Portal.
